ALIGNING SOLUTIONS

With strong partner engagement, SDACH has developed a network of solutions with the goal of assuring that all residents of the North Inland region of San Diego County have sufficient quantities of nutritious foods. These solutions are designed to connect and reinforce each other for greater impact than any single program or intervention could achieve alone.

MEASURING IMPACT

The SDACH is committed to sharing data for making informed, equitable decisions and achieving accountability to the communities we serve. Establishing shared measures to track our collective progress is central to our work. We use population-level data to track outcomes related to cardiovascular protective factors, health equity, and access to care and employ Results-Based Accountability to measure program-level progress on shared measures.
