Why does San Diego Accountable Community for Health (SDACH) have a Privacy Policy?

 

We have a Privacy Policy to keep you informed about how we protect the privacy of personal information we may collect, process, store or transfer when you visit our website.

 

SDACH’s policies exist to ensure that SDACH is transparent and that the individuals accessing the website have the knowledge of information that could potentially be collected. During your visit to the SDACH website, information that is not readily identifiable to an individual may be automatically collected and stored.

 

What type of information does SDACH collect?  How is it used?

 

If you check our website out by browsing and/or downloading information, we may automatically collect and store the following information about your visit:

  • The Internet Protocol (IP) address and domain name used, but not the email address. The IP address is a numerical identifier assigned either to your Internet Service Provider or directly to your computer. We use the IP address to direct Internet track to you;

We use this information to help us improve the content of our Web services and to understand how you are using our service. Additionally, the data we collect helps us route Web traffic and track down viruses and attacks against our site. Our website logs do not identify anyone personally, and we make no attempt to link them with the information about you from other sites.

 

If during your visit to the SDACH website you participate in a survey or send an email, the following additional information will be collected:

What does Personal Information mean to SDACH?  How does choice factor in?

We will not collect personal information about you unless you voluntarily participate in an activity that asks for information (i.e. sending an e-mail) or from your voluntary use of our website (i.e., clicking on a link). If you choose not to participate in these activities, your choice will in no way affect your ability to use any other feature of the company’s website.

 

You may choose to provide the company with additional personal information, such as sending an e-mail to a Company employee or by filling out and submitting an online form located on the company’s Website.

You may recommend changes to your personal information you believe is in error by submitting a written request that credibly shows the error and providing your contact information. We will verify your identity prior to any changes to the personal information on record.  If you believe that your personal information is being used for a purpose other than what was intended when submitted, you may contact us. In all cases, we will take reasonable steps to verify your identity before granting access or making corrections.

We may place cookies (pieces of code) on your device when you visit our website.  These cookies may be used to collect information about how you access and use our website.  You may choose to block the usage of cookies in your browser or mobile device; however you may not be able to fully access our website if you do so.

SDACH is a general audience site and is not directed at and does not target children under the age of 13 years old.  If we obtain actual knowledge that any information we collect has been provided by a child under the age of 13, we will promptly dispose of that information.

SDACH may have links that lead to other websites.  These may include links to websites operated by government agencies, nonprofit organizations, and private business.  As you click on the link and are reroute to another website, you are no longer on the SDACH website and this Privacy Policy will not apply.  When you link to another website, you are subject to the privacy policy of that new website.

We maintain and operate our sites on servers located in the United States. If you choose to use our sites from regions outside the United States that have laws governing data collection and use that differ from U.S. law, you acknowledge and agree that you are transferring personal information outside of that region to the United States and, by providing such information, you consent to the use of your personal data as identified in this Privacy Policy and the transfer of your personal data to the United States.
